The withdrawal ended the Russian's hopes of qualifying for the season-ending WTA Championships and comes one day after Serena Williams, who fell to Dementieva in the final of Moscow last week, also retired injured from her opening round Zurich match.
Schiavone will now face Patty Schnyder in the second round after the Swiss advanced with Williams' withdrawal.
"Obviously I'm very disappointed about the result of the match and disappointed also that I couldn't have made it to Madrid," Dementieva said.
"But I'm still very happy about the tournament last week."
Frenchwoman Amelie Mauresmo exited the Swiss indoor hardcourt tournament after a late collapse allowed Ukrainian Alona Bonadarenko to move past the former world number one 2-6 6-4 6-1.
Mauresmo, who now has a 2-4 record in her last four tournaments after skipping last month's US Open, was atrocious on serve with just 59% accuracy on first serve and nine double faults.
Wimbledon finalist Marion Barotli, who replaced Mauresmo as the French number one earlier this week, had an easier time against Michaella Krajicek beating the Dutchwoman 6-4 6-1.
