The Belgium defender fell awkwardly during the 3-1 defeat to the Black Cats, with the match played on a terrible pitch after heavy rain in Hong Kong.
The 26-year-old former Ajax stopper, who can also play left-back, was stretchered off and will be out of action for around a month.
It makes it highly unlikely that he will feature in Spurs' Premier League season opener against promoted Crystal Palace on August 18.
Kick-off at the Hong Kong Stadium was delayed for half an hour by torrential rain which left surface water on the pitch, and the match was ultimately restricted to two 40-minute halves.
That led Spurs boss Andre Villa-Boas to admit conditions were "extremely poor".
